Before Your Appointment

If you are a new patient, please note that initial intake forms are legally required to be completed prior to the first appointment.

  1. Go to our Client Portal at and create a profile with your CHILD’S first and last name, and an email address for yourself
  2. Complete the INTAKE FORMS in the client portal.

If these steps are not completed prior to your appointment you will be contacted by a member of our team, and your appointment may be cancelled.
